- Last/First Name:
Exactly what's listed on recent forms/documents.

- Date of Birth: Month/Day/Year
For example: 01/12/2016

- Medical Record number:
H________ listed on recent visit forms/documents.

- Email Address:
If no email was given on last visit or on last admission.

Your health information can be accessed through an application such as those used on a smartphone. Depending on the chosen application, you may be able to access information such as your medications, allergies, or lab results. If you
